The CHANGE PAIN Pain Diary is designed to help document and track a person’s pain to support communicating that pain to a healthcare professional. Keeping track of this information can help a doctor determine whether a person is coping with their pain so treatment goals and/or treatments can be discussed and adapted accordingly.

Chronic pain can be managed in a number of different ways.
Most people are aware that there are medicines and therapies (for example, physical therapy, acupuncture) that can relieve chronic pain, with some working better than others in specific types of pain. However, people with chronic pain are not always aware of the role they can have in self-managing their pain.
Working with healthcare professionals to put self-management strategies/techniques into practice can help reduce pain and improve the quality of a person’s life. This can help to put them – not the pain – in control. Our aim is to provide people with chronic pain, and those who support them, with approaches and tools to help with self-management.
